For the 20th consecutive year, Precise Home Care has been recognized on Medicare.gov as one of the top-performing home health agencies in Northeast Florida, scoring above the national average in both patient care measures and patient survey categories. The scores are updated quarterly.
Precise Home Care has earned a 5% reimbursement bonus beginning in 2027 based on our Total Performance Score (TPS) as determined by the Centers for Medicare & Medicaid Services (CMS). The TPS is a composite measure used by CMS to evaluate home health agencies on quality of care, patient outcomes, and patient experience. Achieving a score that qualifies for a positive payment adjustment reflects our team's sustained commitment to clinical excellence and exceptional patient-centered care.
