To get started with Precise Home Care, have your primary care physician (PCP), specialist, or hospital discharge planner contact us with physician orders to begin the process of having you or your loved one assessed for meeting the medical criteria to be admitted under our care while you recover.
Our services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and more — all delivered in the comfort of your home.
How the Process Works
Talk to Your Physician or Specialist
Speak with your primary care physician (PCP), specialist, or attending physician about your need for home health services. Let them know you are interested in Precise Home Care.
Your Provider Contacts Us with Orders
Ask your PCP, specialist, or hospital discharge planner to contact Precise Home Care directly with the necessary physician orders to begin the referral process. We work closely with providers to make this step as seamless as possible.
Clinical Assessment
Once we receive the orders, our clinical team will schedule an in-home assessment to evaluate you or your loved one against the medical criteria required for admission under our services.
Begin Your Care Plan
If eligibility is confirmed, we will develop a personalized care plan and begin services — which may include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and more.
Who May Qualify?
Home health services through Precise Home Care are available to patients who meet Medicare eligibility criteria, which generally includes:
- You are under the care of a physician who certifies your need for home health services
- You require skilled nursing care, physical therapy, speech-language pathology, or continued occupational therapy
- You are considered homebound — meaning leaving home requires a considerable effort
- The home health agency you choose is Medicare-certified (Precise Home Care is Medicare-certified)
